5. Confirmation of cancellation and cancellation policies for contracted services
6. Proof of services issued (hotels, flights, access to events, etc.)
7. At the request of Assist Card, a copy of the entry visa to the destination country
8. Positive PCR result for COVID-19

If you are a resident of Brazil, download, complete, and send the following form to reintegros@assistcard.com: Form "Luggage loss"
